Walmart Covers Ozempic at No Cost, but Only for Diabetes
Walmart will put Ozempic and Mounjaro at $0 for Type 2 diabetes in 2027, skip weight-loss coverage, add NICU leave, and raise starting premiums $1.50.
Walmart will cover Novo Nordisk’s Ozempic and Eli Lilly’s Mounjaro at no cost in 2027 for U.S. associates with Type 2 diabetes. The shots join a new list of more than 250 drugs at $0, including some insulin and inhalers, after a new prior authorization.
Weight loss stays uncovered. The starting medical premium still rises $1.50, to $39.80 per biweekly pay period, as annual enrollment runs through Nov. 6.
Zero-Dollar Ozempic, With a Diagnosis Attached
On Oct. 13, 2026, Walmart posted a Zero Dollar prescription tier for associates that, the company said, “prioritizes medications at lower costs,” with more than 250 generic, brand-name and specialty drugs. Insulin and inhalers are named. Ozempic and Mounjaro are not named on that page. The company separately confirmed both will be $0 for Type 2 diabetes after a new prior authorization, and that it still will not pay for those drugs when they are prescribed for weight loss.
That split is easy to miss in a headline. One reply to the news treated it as free weight-loss shots for staff. It is not. The same molecule that has become a cultural stand-in for dieting is, on this plan, a diabetes drug with a form to fill out.
Donna Morris, executive vice president and chief people officer, put the pitch in a full letter to associates during enrollment season.
We’re adding even more ways for associates and their families to live better through comprehensive and affordable healthcare and benefits.
Donna Morris, Executive Vice President and Chief People Officer, Walmart Inc., in a letter to U.S. associates
A company spokesperson said the changes rest on “years of planning and trying new things,” and that Walmart can improve the package “without slashing benefits or significantly raising premiums.” The $0 diabetes shots are the line that will travel. The diagnosis on the authorization form is the line that will decide who actually pays $0.
More affordable prescriptions. Specialized care. More support for growing families.

Why Weight-Loss GLP-1s Stay Off the Plan
Most U.S. employers already cover GLP-1 drugs for diabetes and refuse them for obesity, so Walmart is joining a crowded file rather than opening a new one. A diabetes-only rule also means the plan pays for the shot after metabolic disease has already become Type 2 diabetes, which is the wait many clinics already know from other commercial plans.
The International Foundation of Employee Benefit Plans, in a June 2 to June 12, 2026, pulse survey, found that 60 percent cover GLP-1s for diabetes only, up from 55% in 2025, 57% in 2024 and 49% in 2023. Coverage for both diabetes and weight loss sat at 36% in 2026, unchanged from 2025 after climbing from 26% in 2023. No responding employer covered the drugs only for weight loss.
EMPLOYER GLP-1 COVERAGE IN IFEBP SURVEYS
| Survey year | Diabetes only | Diabetes and weight loss |
|---|---|---|
| 2023 | 49% | 26% |
| 2024 | 57% | 34% |
| 2025 | 55% | 36% |
| 2026 | 60% | 36% |
Among employers that skip weight-loss coverage, 83% do it with a carve-out in the medical or drug plan, 9% are considering adding the benefit, and 19% used to offer it and stopped. Another 27% steer workers toward direct-to-consumer pharmacies, and 21% point them to an FSA, HSA or HRA. Carey Wooton, associate vice president of education at the foundation, said most employers are “not covering GLP-1 drugs for weight loss but instead focusing on how to support the overall health of their workers.”
Alexis Sova, a clinical advisor at Employers Health, sees the same pattern inside a group-purchasing book of about 500 self-insured clients and $6 billion in pharmacy spend. About 65% of those clients excluded GLP-1s for obesity as of the first half of 2026, up from a roughly even split when the obesity products launched in 2022 and 2023. The remaining 35% mix full member cost share with one-off designs. Starbucks, for its part, stopped covering the drugs for weight loss for benefits-eligible workers in October 2026, while leaving other approved uses in place.
I don’t think anybody wants to close off coverage.
Alexis Sova, Pharm.D., MBA, clinical advisor, Employers Health, after a PBMI 2026 panel
Sova said the brake is cost: a large eligible population times a high price, even though the class has uses beyond the scale, including sleep apnea, the liver disease MASH and fewer cardiovascular events. A $0 copay is also not a $0 claim. Walmart is wiping the associate’s share at the counter for a diagnosed diabetes patient. The plan still pays the rest.
Starting Premiums Rise $1.50 a Pay Period
The starting plan will cost $39.80 per biweekly pay period in 2027, up $1.50 from $38.30 in 2026. Walmart said that increase is smaller than last year’s and that the $39.80 rate is about one-third less than the average premium at other national companies. Full-time and eligible part-time associates can enroll. A spokesperson said workers who average more than 30 hours a week qualify.
THE STARTING PLAN IN NUMBERS
- $39.80: 2027 starting medical premium per biweekly pay period.
- $1.50: Increase from the 2026 starting rate of $38.30.
- 1.6 million: U.S. associates as of the end of fiscal 2026.
- 92%: Share of the U.S. workforce that is hourly.
Walmart employed about 2.1 million associates worldwide at the end of fiscal 2026, with about 0.5 million outside the United States, and 39% of the U.S. base under age 30. That mix is why a $1.50 bump is not an abstract finance slide. It is a line on a biweekly check for a mostly hourly workforce that already buys groceries at a 10% associate discount and, in stores and clubs, gets a free Walmart+ or Sam’s Club Plus membership.
The new drug tier sits on top of benefits the company already advertises under most medical plans: 20 no-cost therapy sessions a year, $0 virtual visits for urgent care, primary care, mental health, physical therapy and digestive health, and up to $20,000 for fertility services, adoption or surrogacy. Those items did not start this enrollment cycle. They are the floor the $0 list is being built on.
Parents Can Stack NICU Leave to 28 Weeks
The benefit that is hardest to spin as a pharmacy maneuver is the new paid NICU leave. Walmart said one in ten births require a neonatal intensive care stay. Beginning Nov. 17, 2026, eligible associates whose children are admitted during the first 28 days after birth, and who are still in inpatient care, can take a maximum of 12 weeks of paid NICU leave. That time stacks on maternity leave, parental leave and other paid time off. Frontline associates can reach up to 28 weeks of paid leave when a birth includes a NICU admission.
The drug list and the rare-conditions clinics wait until Jan. 1, 2027. The NICU policy does not. It lands in the middle of the holiday staffing season, which is when a lot of hourly parents would otherwise be choosing between a paycheck and a hospital bassinet.
THE 2027 BENEFITS CALENDAR
- Oct. 13, 2026: Morris sends the enrollment letter and Walmart posts the three changes.
- Nov. 6, 2026: Annual enrollment closes.
- Nov. 17, 2026: Paid NICU leave begins, up to 12 weeks, stackable to 28 weeks for frontline associates.
- Jan. 1, 2027: The Zero Dollar drug tier and the Rare Conditions Center of Excellence take effect.
Leave is also the piece of the package that does not depend on a prior authorization. If a newborn is in the unit, the clock is the medical record, not a diabetes code.
Rare Conditions Join Cancer at Mayo Clinic
Walmart is adding a Rare Conditions Center of Excellence for associates and dependents on most of its medical plans, with no-cost access to specialists for multiple sclerosis, Parkinson’s disease and ALS. Mayo Clinic and Cleveland Clinic are named partners. People being evaluated for, or already diagnosed with, one of those conditions can get an in-person evaluation and treatment review. Travel is covered for the patient and an adult caregiver, as it is on the company’s other centers of excellence.
The rare-conditions clinic is an expansion of a cancer program that already sent associates to the same hospitals. Tasheka Saunders, a distribution-center administrative assistant, went through that cancer path in 2023 after she was told she needed a full mastectomy. Mayo Clinic gave a second opinion, changed the plan, and treated her. In Walmart’s write-up she said, “I am alive today because of Walmart’s Centers of Excellence.” She is now cancer-free, per that account.
A company spokesperson said the $0 drug list also includes hemophilia medicines and a multiple sclerosis drug, each of which can cost thousands of dollars a month. Those are not lifestyle prescriptions. They are the kind of specialty claims that, unlike a weight-loss fill, hit a small number of families with very large bills. Putting them at $0 is a different bet from putting Wegovy on the menu for anyone with a BMI high enough to qualify.
Who Can Enroll Before Nov. 6
Full-time and eligible part-time associates can take medical coverage that starts at $39.80 per biweekly pay period, and a spokesperson said workers who average more than 30 hours a week can enroll. Annual enrollment runs through Nov. 6. The Zero Dollar tier and the rare-conditions clinic apply to people on most Walmart medical plans. GLP-1s at $0 require Type 2 diabetes plus the new prior authorization. NICU leave requires a child admitted in the first 28 days after birth who is still an inpatient.
WHO CAN USE THE NEW BENEFITS
- Medical plans: Full-time and eligible part-time associates, with enrollment open through Nov. 6.
- $0 GLP-1s: Type 2 diabetes, plus a new prior authorization; weight-loss use stays out.
- Rare-conditions clinic: Associates and dependents on most plans who are being evaluated for, or diagnosed with, MS, Parkinson’s or ALS.
- NICU leave: Eligible associates whose newborns are admitted in the first 28 days and remain in inpatient care, starting Nov. 17, 2026.
The CDC’s National Diabetes Statistics Report estimates 40.1 million people with diabetes in the United States as of 2023, about 12.0% of the population, including 29.1 million with a diagnosis. Type 2 accounts for about 90% to 95% of diagnosed cases in adults. Another 115.2 million adults have prediabetes. Walmart has not said how many of its 1.6 million U.S. associates have Type 2 diabetes, and applying the national rate to that headcount would be a guess. The prior authorization is how the company will count them in practice.
Walmart already runs a separate diabetes and metabolic program. Wayne Jaco, a private fleet driver, joined it after a Type 2 diagnosis, used a glucose monitor and a smartwatch, and lost 50 pounds in the first year, with better blood pressure, cholesterol, insulin resistance and liver function, according to the company. He said he is “almost at the point of reversing my type 2 diabetes.” Free Ozempic or Mounjaro would stack onto that program for people who clear the new approval, not replace it.
Insulin, Inhalers and the Rest of the 250
The official list is “more than 250” drugs, described as generic, brand-name and specialty, with “some” insulin and “some” inhalers called out. Walmart has not published the 250 names. A $0 insulin fill would matter on an hourly paycheck even if the GLP-1 news did not exist. So would a $0 inhaler. Those products do not carry the same plan-level risk as an open-ended obesity benefit, which is why they can be advertised in the same sentence as Ozempic without opening the same checkbook.
WHAT WE KNOW
- The diabetes shots: Ozempic and Mounjaro will be $0 for Type 2 diabetes after a new prior authorization, the company confirmed.
- The obesity line: Novo and Lilly GLP-1s prescribed for weight loss stay uncovered.
- The rest of the tier: More than 250 drugs, including some insulin and inhalers, plus specialty products a spokesperson said include hemophilia and MS medicines.
WHAT IS UNCONFIRMED
- The 250 names: Walmart has not released a public formulary for the Zero Dollar tier.
- The plan’s net price: The company has not said what it will pay per GLP-1 fill after rebates.
- A later obesity add-on: Nothing in the Oct. 13 materials commits Walmart to covering Wegovy, Zepbound or any other weight-loss GLP-1 in 2027.
For associates who wanted the shot for weight loss, the 2027 plan still points them to cash-pay pharmacies and tax-advantaged accounts, the path a growing share of employers already recommend. For associates with Type 2 diabetes who can get the new authorization, the pharmacy counter price drops to $0 on Jan. 1, 2027. Annual enrollment runs through Nov. 6. Paid NICU leave begins Nov. 17. The drug list and the rare-conditions clinics open with the new year for people whose plans, and whose diagnoses, match the rules.
