Dylan Cease’s Shoulder Inflammation Ends a Cy Young Season
Dylan Cease’s shoulder MRI showed inflammation, not a tear, after he pitched through the ache since a 120-pitch July shutout.
Dylan Cease will miss his Wednesday start in Baltimore after an MRI found right shoulder inflammation. The scan showed no tear, and the Blue Jays do not expect the problem to follow him into 2027.
He is still on the active roster. Manager John Schneider has not ruled out Cincinnati this weekend, but only if Cease feels like himself and the games still matter. After a 4-3 loss on Monday, Toronto is 3.5 games behind the White Sox for the last American League wild-card spot, with five games left and no ace lined up.
The MRI Cleared 2027 and Closed Wednesday
Cease rejoined the club in Baltimore on Monday and walked reporters through the results. The MRI, taken Saturday night in Toronto after he was scratched from Wednesday, showed only inflammation in his right shoulder. Nothing structural. Nothing that points to an operation.
It’s definitely a bit heavy and achy, just not normal. At the same time, it didn’t feel like it was torn. It’s not something I need surgery for or anything like that, it’s just inflammation, that middle ground.
Dylan Cease, Blue Jays pitcher, speaking in Baltimore
He would not call it painful. He also would not pretend the last month was normal. Fastball speed had been sliding, command went with it, and Friday in Texas he lasted 2 2/3 innings. The club kept him off the injured list. That is a roster choice, not a medical all-clear for another start this week.
Schneider’s bar for using him again is narrow. Cease has to feel better after the days down, and the game has to mean something in the standings. Those two things have to line up. After Monday, the second one is already slipping.
A Heavy Shoulder Since the Boston Shutout
The shoulder did not arrive on Friday. Cease said the joint first felt off around his complete-game shutout in Boston on July 25, a 120-pitch, one-hit night in which he struck out 12. He kept taking the ball. The heat that had flirted with 100 mph early in the year settled into the mid-90s, then lower.
That is the part the diagnosis quietly rearranges. A Cy Young line built on 29 starts now has a two-month footnote: he was pitching through a heavy arm while the club kept handing him 100-pitch nights. He topped 100 pitches in 16 of those 29 starts. The MRI is the ending of that stretch, not the beginning of it.
THE TWO MONTHS HE PITCHED THROUGH IT
- May 25, 2026: Hits the 15-day injured list with a left hamstring strain, his first trip to the IL as a Jay.
- June 9, 2026: Activated after a rehab outing with Buffalo.
- July 25, 2026: Throws a 120-pitch complete-game shutout at Boston, 9 innings, 1 hit, 12 strikeouts, and first feels the shoulder.
- Sept. 18, 2026: Lasts 2 2/3 innings at Texas with six walks and a career-slow four-seamer at 90.0 mph.
- Sept. 19, 2026: Flies to Toronto for an MRI of the right shoulder.
- Sept. 21, 2026: Tells the club in Baltimore the scan is inflammation, with no surgery and no expected hit to 2027.
He described the joint as heavy and achy since late July, and he kept going because, in his words, most pitchers play with something. The last start changed the math. He said he almost felt as if he were hurting the team.
How a 90 mph Fastball Ended His Start
Friday at Globe Life Field was the shortest outing of his season and the one that forced the scan. Cease faced 17 hitters, walked six, allowed three hits and four runs (one earned), and recorded three strikeouts. Toronto lost 7-1. His four-seam fastball averaged 93.6 mph, more than three ticks under his season mark, and he threw the slowest four-seamer of his career at 90.0 mph.
That is not a blip on a radar gun. A pitcher who lived in the upper 90s in April cannot live at 90 in September and still miss bats the same way. He said the velocity drop had been concerning, but he was still getting outs until that night, when the walks piled up and the outing collapsed in the third.
The Blue Jays had already announced he would not start Wednesday before the full MRI language was public. Schneider had framed it as fatigue, not pain, and said he was not expecting anything wild. The scan matched that hope. It also confirmed why the fastball had been leaking for weeks instead of bouncing back between starts.
Schlittler Now Pitches the Cy Young Alone
Cease still has a top-five Cy Young case. He will not get another start to move the counting stats. Yankees right-hander Cam Schlittler, at 14-6 with a 1.94 ERA through 190 2/3 innings after his Sept. 20 outing in Arizona, now finishes the year without a live challenger on the mound.
Cease’s own ledger is the best season of his career and the one Toronto bought. He is 11-6 with a 2.40 ERA, 169 innings and 239 strikeouts in 29 starts, a 1.10 WHIP, and 12.7 strikeouts per nine. That ERA ranks second in the American League. The strikeout total ranks second in the AL and fourth in Blue Jays history, behind Roger Clemens (292 in 1997 and 271 in 1998) and Robbie Ray (248 in 2021). Milwaukee’s Jacob Misiorowski (247) and Cleveland’s Gavin Williams (244) are the only pitchers with more punchouts in the majors.
AL CY YOUNG, FINAL FULL LINES
| Pitcher | W-L | ERA | IP | SO | BB |
|---|---|---|---|---|---|
| Cam Schlittler, Yankees | 14-6 | 1.94 | 190.2 | 234 | 47 |
| Dylan Cease, Blue Jays | 11-6 | 2.40 | 169.0 | 239 | 78 |
Schlittler holds innings, ERA, walks, and wins. Cease holds strikeouts and the tougher opponent batting average,.180, which leads the AL. Voters who already preferred the Yankees’ workload were not going to be talked out of it by two more Cease starts. They also will not have to weigh those starts. Cease finished second in the AL voting in 2022, when he went 14-8 with a 2.20 ERA for the White Sox. A second runner-up is still in range. A first trophy is not, unless ballots treat a shut-down ace as equal to a pitcher still taking the ball.
Toronto’s 3.5-Game Hole Has No Ace Left
Monday’s 4-3 loss at Camden Yards dropped the Jays to 77-80. Pete Alonso hit two home runs. Trey Yesavage, back from the injured list, threw two innings in a short appearance. Max Scherzer is lined up Tuesday against Chris Bassitt. Nobody has been named for Wednesday, the night that was Cease’s.
Chicago is 80-76 and owns the head-to-head tiebreaker after going 5-1 against Toronto. Cleveland is 81-75, a game up on the White Sox in the Central, and would lose a tiebreaker to the Jays, which only matters if both clubs back into the same number. Houston is 77-79. Toronto’s remaining slate is Baltimore on Tuesday and Wednesday, an off day Thursday, then three at home against Cincinnati.
WHAT IS STILL ON THE SCHEDULE
- Tuesday, Sept. 22: Scherzer at Baltimore opposite Bassitt, 6:35 p.m. ET.
- Wednesday, Sept. 23: Starter TBA in the game that was Cease’s turn.
- Thursday, Sept. 24: Off day, then home for Cincinnati.
- Friday-Sunday, Sept. 25-27: Reds at Rogers Centre, the only window Schneider left cracked.
Even a 5-0 finish gets Toronto to 82-80. The White Sox, with six games left against Kansas City and Colorado, would have to collapse for that to matter, and they would still hold the tiebreaker in a dead heat. Consecutive sweeps were the long-shot path when Cease was still in the picture. Without him, the rotation that has lived with TBA cards has to cover the last five on fumes.
Year One of $210 Million Stops at 29 Starts
Cease signed with Toronto on Dec. 2, 2025, on a seven-year, $210 million free-agent deal, the largest open-market contract in club history. The structure includes a $23 million signing bonus, a $22 million salary in 2026 with $10 million deferred, then $30 million in 2027 that steps down by $1 million a year. Another $64 million is deferred through 2046. The union’s present-value figure sits near $184.6 million.
Year one is the season the front office wanted on the field. He made the All-Star team, won AL Pitcher of the Month in July, and posted 5.6 WAR and a 2.47 FIP. The Blue Jays went 18-11 in his 29 starts. Over his last 15 turns he was 7-3 with a 2.09 ERA and 121 strikeouts in 90 1/3 innings, which is the pitcher they paid for, right up until the fastball fell off the table.
THE DEBUT YEAR, CLOSED AT 29 STARTS
- The line: 11-6, 2.40 ERA, 169 innings, 239 strikeouts, 78 walks, 1.10 WHIP.
- The ranks: Second in the AL in ERA and strikeouts, first in opponent average at.180.
- The workload: 16 of 29 starts at 100-plus pitches, including the 120-pitch shutout that started the shoulder complaint.
- The next check: $30 million salary in 2027, the first year the deal really steps up.
He has already named the adjustment he wants next year: more rest between starts, different spots to back off. That is a 2027 problem he is allowed to have because the MRI stayed in the middle ground. Shoulders still carry longer questions than elbows on a seven-year clock, and he has a Tommy John surgery from high school on the résumé, but he had not landed on the major-league IL for his throwing arm as a pro. The hamstring in May was the other side of the body. This is the first time the club has had to stop him for the arm itself.
Schneider Will Only Use Him If the Week Still Matters
The official stance is limbo. Cease said he and the club are still drafting a plan. Schneider said the perfect case is that Cease feels like himself and the standings still give the start weight. A first-place club in October would run him out there on inflammation. A club that is 3.5 out with five to play, and that already lost the opener in Baltimore, does not get that excuse.
Shutting him down is the clean reading of the MRI, the velocity, and the math. It is also the reading that treats 2027 as the season this contract was always going to be judged on. He will finish among the top five in Cy Young voting for the third time. He will not add to 239 strikeouts unless Cincinnati suddenly becomes a must-win series that Toronto has not earned.
For now the Blue Jays have a healthy-enough ace on the roster who is not taking the ball, a rotation already using bullpen days, and five games that have to go right for a chase that needed him on Wednesday. The scan was the good news. The calendar is the rest of it.
